Dan -----Original Message ----- From: Tamara Weaver Sent: Wednesday, July 09, 2003 11:40 AM To: Marty Wynne; Jeanine Faria Cc: Daniel Peddycord; Teri Maerki; Elaine Severson Subject: Request from Dan Peddycord for contingency transfer Per Dan's discussions with Mike and Marty, I have attached a request for a transfer from contingency in Fund 260 (Healthy Start)for a temporary continuation (to August 31) for the .5 FTE Health Educator position costs (Erin Swanson). Erin had been informed of a layoff as of June 30 for this portion of her job (i.e. 1/2 of her was laid off). Healthy Start requested that she continue for two months with Healthy Start. I have filled out forms for personnel to let them know that Erin's .5 MOA position (regular)continues for the entire budget year, but the Health Educator position will be a two months temporary position in Fund 260. Please let me know if I need to do this diferently. I am also preparing a request for Elaine Severson's ESD contract which will not end as expected, but will continue at a lower amount (thus Heather Doorn continues at a lower FTE). I cannot finish that request yet due a question on the ESD amount of contract, but will try to get that done this week. 1 HEALTH DEPARTMENT FUND 260 HEALTHY START Manager - Elaine Severson REQUEST FOR FY04 BUDGET TRANSFER FROM CONTINGENCY FOR HEALTHY START DESCRIPTION I ACCOUNT 1PROJECTI AMOUNT I TOTALS FROM: Contingency: 260-2000-501.97-01 4,110 To: Expenses - INCREASE FOR ERIN SWANSON FOR 2 MONTHS TEMPORARY CONTINUATION Personnel Health Educator 260-2000-441.16-22 FND 260 2,600 Life/Dis insurance 260-2000-441.21-10 FND 260 50 Health/Dental Ins. 260-2000-441.21-50 FND 260 700 FICA/Medicare 260-2000-441.22-01 FND 260 200 PERS 260-2000-441.23-01 FND 260 510 UI ins. 260-2000-441.25.01 FND 260 20 Worker's Comp 260-2000-441.26-01 FND 260 30 Total personnel 4,110 STAFF: Erin Swanson 50% Health Educator in Fund 260 50% MOA in HBCS Authorization # This position was eliminated in the FY04 budget process.
