Also present was Tom Anderson, County Administrator. Attending for a portion of the meeting were David Inbody and Kacey Burgess, Health Department; Whitney Hale, Communications; Peter Russell, Community Development; David Givans, Internal Auditor; Danielle Fegley and Kathleen Hinman, Human Resources; and James Lewis, Properties. No other citizens or representatives of the media were present. Chair Unger opened the meeting at 1:30 p.m. 1. Discussion of Recommended Changes to Deschutes County Employee Benefit Plan. Kathleen Hinman reviewed the Plan's recommended changes, as presented to EBAC. The benefit consultants have been working on the plan with them. They feel there should be a change in the PPO. Last year this was done to see an increase in saving and have a larger network and provider access. A year later, they did not see the excepted savings. Part of this has to do with claims distribution, as there were some high ones this year. Also, there was a change in providers in the Aetna network; they have lost some of them. The plan was to increase or maintain the provider network. There will need to be new ID cards for everyone if this change is made. Stop loss was changed while they were looking at the options, which will be broadened. They identified Sunwise Financial for this, as their rates are better and there is a refund option. The dependents audit is going to be done during open enrollment, with a third party. This will be provided to all employees for input. By the end of the year, they hope to have employees substantiate their dependents on the plan. Mr. Anderson asked how they will know the employees have seen this. Ms. Hinman said that it will be mailed to all homes, will be on the website and provided at the departments through meetings. Employees will be required to return the information in a timely fashion. They really need to have what the State defines as a legal spouse. There may be ways for employees to access this information through records searches. Commissioner DeBone asked if there are actually extra dependents being claimed, and Ms. Hinman responded that there are. This is a fiduciary responsibility of the County. Ms. Fegley said there is a lot of risk if someone is not officially a dependent. Chair Unger asked about those who are living together, but are not married. Many of them are committed to the relationship. There must be ways that other areas have dealt with this, and he wants to be fair. Commissioner DeBone said there need to be clear lines, and he sees this as being very separate. Chair Unger would like to see more discussion on this issue. Ms. Hinman stated that there are State guidelines. There have been large sums of money paid out that should really be the responsibility of the employee. The Affordable Care Act has requirements, too, relating to identification. They have found 122 errors in the County system. Mr. Anderson wants to be sure people are clear on the documentation that is needed. Ms. Hinman talked about retiree COBRA rates. There would be a 9.3% increase rate with this change of providers; compared to 14% if they did not change. Minutes of Board of Commissioners' Work Session Wednesday, June 29, 2016 Page 2 of 6 There is a new pharmacist, since Ron Alvig retired. Nathan Rouse is the new pharmacist who starts on August 1. He is excited about the model being used. There is a vacancy in the DOC as well. They have looked at a couple of different staffing models. Appointments are getting behind at this point. Ms. Hinman is working with Medcor on options. It could remain the same, or the second option is that they could replace the position with a nurse practitioner or physician's assistant, which would increase the number of appointments available by 60. There would still be physician oversight. The need for an actual physician is very low and usually involves a medical specialist. They can work with members to fmd a more suitable physician. Another option is having a doctor present for part of the time. However, there is not a lot of interest from doctors for part-time work of this type. They think the second option is the best way to address capacity needs. Mr. Anderson stated that it is significant that they reached capacity, as that is an important milestone. He feels that being able to increase availability is important. The Commissioners were supportive of the second option. DEBONE: Move approval of the overall recommendations as stated. UNGER: Second. David Inbody and Kasey Burgess presented a grant application relating to a referral coordinator to help with outreach, referral services and education to provide better client care. This will help with the movement toward a shared community health record in the tri -County area. Administrative staff tries to do this work, but it is difficult for them to do this properly on top of their normal duties. Mr. Inbody said that most of the local agencies see the value and assume Deschutes County would be the applicant. Some organizations have referral teams. This would not just be the County's, but part of a regional plan. Tom Anderson asked if this is a County position. Ms. Burgess stated that as a pilot program, it would best be imbedded in Deschutes County. After that, Pacific Source or another provider may end up being responsible. Chair Unger would like to see the referrals be handled better so as to not waste the time and resources of the County, hospital or doctors. An example of the service users are pregnant women and home health services. Mr. Inbody stated that the cost of the position is $157,000. The grant request is $150,000, which is a threshold number for Pacific Source. The Department would cover the balance. DEBONE: Move approval. UNGER: Second. VOTE: DEBONE: Yes UNGER: Chair votes yes. Peter Russell asked if the Board wanted to call up the Airport Master Plan on appeal. He has not heard from anyone else and the appeal period ends at 5:00 today. Chair Unger asked what issues could be appealed on this matter, and whether there are any issues of errors. Mr. Russell said that no one has participated yet, but the public can still be heard. There were no substantive concerns raised. The uses and traffic issues are not changing, and other things are not under the purview of local governments. The Hearings Officer said that the City is proceeding legally. Mr. Russell does not know why the Board would want to call this up. Nineteen people testified, with most objecting to helicopter operations that are already allowed. Commissioner Unger is concerned about the impacts, and would like to see some solutions to conflicts. Commissioner DeBone does not have a feel for why an appeal would help. Mr. Russell stated that the City has another whole land use action to complete. Minutes of Board of Commissioners' Work Session Wednesday, June 29, 2016 Page 4 of 6 James Lewis asked about the heliport. Mr. Russell said there is a site plan but this use was identified in 2002. Mr. Lewis stated that they may be asking for access over County -owned property. This may trigger additional land use action. Mr. Russell said that Airport management is sensitive to potential problems, but they can only request the users to be more careful, as aircraft in the airspace is not under the Airport's or local governments' control. Commissioner DeBone does not see a reason to appeal this. Chair Unger does not think this is an effective time to do anything, and wants to avoid a punitive process. He wants the users to do all they can to keep the neighborhood happy. He asked for a copy of the 'good neighbor' brochure. An application for a discretionary grant for a Newberry event was sent to Commissioner DeBone. He said it is a good summertime gathering for the community, and he would like to grant $500. He does not want to create precedence for others. He noted that he attended some of it last year, and knows there are many similar events in the County. COVA acknowledged the event, and it is on event calendars for the area. Mr. Anderson said that statewide MS is the beneficiary and Commissioner Baney prefers that the beneficiary be more local. Commissioner DeBone noted that they may try to obtain grant funds again next year. Chair Unger said the permit and insurance alone are $400. It was indicated that Eagle Crest Resort has pulled out of COVA. Chair Unger had questions about the resorts advertising that taxes and fees are `taxes', so it appears that the County is charging a higher amount than it is. Some of these costs are really various resort fees. Mr. Anderson stated that he will bring this up to COVA, but is not sure how the resorts will react. It is probably broken out in detail on the final bill, Chair Unger does not like them lumping it all together and feels that it makes the County look greedy. Many changes are in response to updated State and Federal requirements, and others are recommendations that were made by Davidson Benefit Planning (Davidson) after completing their financial review of the plan. These changes are scheduled to take effect when the plan renews on August 1, 2016. I have provided a more detailed explanation below on a few of the changes. 1. Chance in PPO. Move from current Participating Provider Organization (PPO) network, Aetna, to First Choice. This change is only for accessing the PPO network. Davidson went to multiple PPO networks to have our claims repriced. All three, BlueShield, First Choice, and PacificSource returned results with savings higher than Aetna. This move is expected to: • Increase savings from providers • Maintain larger network — geographically • Increase access to network providers New ID cards will be mailed to all plan members due to the change in PPO. 2. Change in Stop Loss Carrier. Davidson reviewed the plan's stop loss levels and determined that the plan is adequately insured for stop loss coverage at its current levels. However, Davidson did bid the coverage as there is room for improvement in the contract terms and rates. Of the five responses, the recommendation is to move to Sun Life Financial for stop loss coverage. Their bid is competitive, included no lasering at renewal, and a stronger refunding mechanism. 3. Dependent Audit. During the 2016 Open Enrollment, the plan will be completing a "soft audit" of dependents with a full dependent audit to be completed at the beginning of 2017 by a third party. 4. Retiree and COBRA health plan rates. With the above mentioned changes, the plan would renew with a 9.3% increase to rates for the 2016/17 plan year rather than a 14.8% increase. 5. New Pharmacist selected for DOC Pharmacy. The pharmacist will begin the onboarding process in mid July with the expectation that he will assume the full responsibilities by August 1, 2016. 6. Update on staffing model in the DOC Clinic. Add Lactation Support and Counseling within the Breast pump benefit in the Covered Charges section. These charges will be payable subject to the Preventive Care Benefits (at the Preferred Provider benefit level for both Preferred and Non - Preferred Providers.) The time limitation period for obtaining the breast pump must be removed from the Breast Pump benefit. Benefits for Obesity Interventions will now have a 26 visit maximum. Revise the Obesity Exclusion in the Plan Exclusion section to align with this new benefit. Update the Nutritional Education benefit to allow for new mandated benefit of Obesity Interventions (for adults ages 18 years and older and with a BMI (body mass index) of 30 kg/m2 or greater). The Plan will Utilize First Choice as the Participating Provider Organization (PPO) Update list of Preventive Care Drugs with No Cost Share. The plan document will be updated with a link to the list of US Preventive Service Task Force recommended medications as this will keep the plan document current when changes are made to the list off plan year. Update the covered charges description. The current description is very specific and may not represent actual billing processes currently being used. The NBE sponsors La Pine's Frontier Days Talent Show, then puts the winner on a main stage performance at the event, three weeks later. The NBE was able to donate almost $3000 to OR NMSS last year. If we reach the $10K goal this year, that money will benefit people like myself with multiple sclerosis, as we strive to apply science and research to battling this crippling disease. ** [Multiple sclerosis, an unpredictable, often disabling disease of the central nervous system. It is both a neurologic and auto -immune disease. Nerve damage interrupts the flow of information between the brain and body. Symptoms range from numbness and tingling to blindness and paralysis, eventually affecting the ability to stand and walk. The progress, severity and specific symptoms of MS in any one person cannot be predicted, but advances in research and treatment are moving us closer to a world free of MS. Some research clues are that most people with MS grow up in middle latitudes of the globe, are diagnosed between the ages of 20 and 50, have a genetic susceptibility, and almost three times more women than men are diagnosed with the disease.
