This Fourth Amendment to Oregon Health Authority 2019-2021 Intergovernmental Agreement for the Financing of Public Health Services, effective July 1, 2019, (as amended the "Agreement"), is between the State of Oregon acting by and through its Oregon Health Authority ("OHA") and Deschutes County, ("LPHA"), the entity designated, pursuant to ORS 431.003, as the Local Public Health Authority for Deschutes County. RECITALS WHEREAS, OHA and LPHA wish to e.g. modify the Fiscal Year 2020 (FY20) Financial Assistance Award set forth in Exhibit C of the Agreement. WHEREAS, OHA and LPHA wish to e.g. modify the Exhibit J information required by 2 CFR Subtitle B with guidance at 2 CFR Part 200; NOW, THEREFORE, in consideration of the premises, covenants and agreements contained herein and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties hereto agree as follows AGREEMENT 1. RECOMMENDATION & ACTION REQUESTED: Staff recommends approval of Resolution 2019-054 appropriating new grant funds. BACKGROUND AND POLICY IMPLICATIONS: As part of the Public Health Intergovernmental Agreement #159809 amendment #4, the Oregon Immunization Program is making funds available to Deschutes County Public Health to conduct targeted outreach efforts and to provide Hepatitis A vaccinations to high-risk individuals, including unstably -housed and homeless populations. The purpose of the funding is to prevent Hepatitis A outbreaks, such as those recently seen across the United States among at -risk individuals. FISCAL IMPLICATIONS: The funding to be requested is one-time funding in the amount of $40,000 to be allocated and spent November 1, 2019 to June 30, 2020. These funds will be used for the purchase of immunization supplies and to contract with a part-time Outreach Coordinator November 1, 2019 -June 30, 2020.
