A Resolution Accepting New Grant Funds,
and Increasing Appropriations within the * RESOLUTION NO. 2021-004
2020-21 Deschutes County Budget
WHEREAS, Deschutes County's Health Services has been awarded $20,741 in new
revenue from the Sustainable Relationships for Community Health (SRCH) grant, and
WHEREAS, it is necessary to approve acceptance of the Sustainable Relationships for
Community Health grant, and
WHEREAS, ORS 294.471 allows a supplemental budget adjustment when authorized by
resolution of the governing body, and
WHEREAS, it is necessary to increase appropriations by $20,741 in Program Expense to
accommodate this request, and
BE IT RESOLVED BY THE BOARD OF COUNTY COMMISSIONERS OF
DESCHUTES COUNTY, OREGON, as follows:
Section 1. That the following grant revenue be budgeted in the 2020-21 Deschutes
County Budget:

AWARD ANNOUNCEMENT: Sustainable Relationships for Community Health (SRCH) - 2020-21

Thank you for submitting an application for SRCH funding to support Adaptation to virtual delivery
for Prevent Diabetes Central Oregon and Living Well Central Oregon. Health Promotion and Chronic
Disease Prevention (HPCDP) staff has reviewed the Deschutes County SRCH application and is
pleased to provide the full amount of funding ($20,741) requested to support your project work. We
are excited to support this work in your community.
The funding for this work will be disbursed in monthly payments via the Sustainable Relationships for
Community Health Program Element 04 (SRCH PE04). HPCDP will initiate the process of amending
the OHA-PHD Intergovernmental Agreement (IGA) PE04 for a start date of 1/1/2021. The fiscal
reporting process will occur via the standard submission of quarterly revenue and expenditure reports.
Specific to your project we would like to highlight the following for your team:
• We are pleased that Deschutes County works collaboratively with its regional partners and
LPHAs in Crook and Jefferson Counties and is proposing a regional approach for this project.
Crook and Jefferson Counties are also receiving funding as requested.
• We will be particularly interested to learn with you as your region implements Welld as this
platform not only with help with sustainability of self -management programming, but the CDC
is very interested in communities which are utilizing this platform.
• As follow-up to our pre -application call with you, we mentioned Omada as a potential resource
- https://www.omadahealth.com/. In addition, we understand that Canary Health is offering
services in Oregon and is newly exploring services for Medicaid beneficiaries -
https://www.canaryhealth.com/.
As described in the funding opportunity, HPCDP and/or our contractor(s) will be providing support
and technical assistance that your team may wish to request. We will be in touch in the coming weeks
to both ask you what you may need and describe what we can make available to support your project
success.

Consideration of Board Signature of Resolution No. 2021-004 Accepting the Sustainable
Relationships for Community Health (SRCH) Chronic Disease Prevention and Management
Amid the COVID-19 Pandemic Grant and Increasing Appropriations Within the 2020-21
Deschutes County Budget
RECOMMENDATION & ACTION REQUESTED:
Staff Recommends Acceptance of Resolution 2021-004, Accepting the SRCH Grant and
Increasing Appropriations with the Health Services Fund.
BACKGROUND AND POLICY IMPLICATIONS:
Deschutes County Health Services (DCHS) provides two regional programs focused on
chronic disease prevention and self -management: Living Well Central Oregon (LWCO) and
Prevent Diabetes Central Oregon (PDCO). The grant resources will enable DCHS to
upgrade these programs to navigate the challenges presented by the COVID pandemic
and improve service delivery.
More specifically, funding will:
1. Improve the virtual delivery of LWCO workshops by:
a. Training two local Master Trainers in the virtual certification offered by Self -
Management Resource Center,
b. These two Master Trainers will then train local leaders and coach them for
offering virtual workshops,
c. Covering printing and mailing fees incurred to ensure participants have the
resources available to them in hard copy format, and
d. Providing curriculum toolkits to participants without access to virtual tools.
2. Transition PDCO to an upgraded data management system, ensuring appropriate
licensure and training of staff.
This grant opportunity aligns with the Regional Health Improvement Plan priority area
"Promote Enhanced Physical Health Across Communities." This priority area aims to
achieve improvements across the following metrics: age -adjusted rates of chronic disease
and prevalence of risk factors for preventable disease.
LWCO programs are evidence based workshops developed at Stanford University and the
Self -Management Resource Center. They are proven to improve a broad range of health
outcomes and reduce healthcare costs. PDCO provides the National Diabetes Prevention
Program, which was developed by the CDC based on randomized controlled trial that
found lifestyle change reduces the risk of type 2 diabetes by 58%.
FISCAL IMPLICATIONS: Grant provides $20,741 for Fiscal Year 21.
