Memorandum of Understanding and Agreement
Links 4 Health: Health Integration Project
Collaborating Organizations include: Cascade Healthcare Community, Inc., Deschutes
County Health Services, Jefferson County, Crook County, Deschutes County, Mosaic
Medical, Best Care Treatment Services, National Alliance on Mental Illness of Central
Oregon, Accountable Behavioral Health Alliance, Deschutes County Chemical
Dependency Organization, Central Oregon Individual Health Solutions, Lutheran
Community Services Northwest, HealthMatters of Central Oregon, LaPine Community
Clinic, Cascade Peer and Self Help Center, and Volunteers in Medicine

PURPOSE
This Memorandum of Understanding serves as a method to build and strengthen the
collaboration between Links 4 Health: Health Integration project (L4H:HIP) participating
organizations. The purpose of this agreement is to highlight the roles of the collective
agencies to enhance the services provided to individuals living with health disparities and
other mental health conditions within Crook, Deschutes and Jefferson counties. This
agreement will be effective when signed by all L4H:HIP collaborating organizations.
"Links 4 Health" is the Care Coordination Pathway project facilitated by the neutral
convener, HealthMatters of Central Oregon. Care Coordination Pathways are tools used
to shift the focus of health and human service providers from activities to outcomes and
to support the client's treatment team. Pathways are unique in that the outcomes are
tracked at the level of the individual. The Pathways (developed by the treatment team)
provide a step by step guide for the care managers (nurse, social worker, Community
Links Specialist) to work through a specific client problem focusing on achieving a
positive result. Each step of the Pathway addresses a clearly defined action towards
problem resolution. The Pathways utilized can span any health or social service issue.
This MOU refers specifically to the Behavioral Health Integration project, which is part of
the overall integrated delivery system named "Links 4 Health."
AH L4H collaborating agencies agree to:
1. Coordinate efforts between community partners (Health departments, health
systems, providers, safety net clinics, and other health and human service
agencies and stakeholders within the tri -county region) to facilitate the L4H
program and serve as regional resources for the L4H program;

2. Share information such as: current regional client population data (HIPAA
compliant), design, development and implementation concepts, as well as, provide
input on program benchmarks and program evaluation data;
3. Work together to develop a Transitional Board (Exhibit A). This Board will be
chartered for one year following date of implementation of this agreement and
charged with oversight of policy, outcomes, strategic planning and approving the
work of the Stakeholder Committee and the Project Development Committee. This
Board will also be charged with the conceptual development of a single point of
accountability for the region.
4. Work together to develop a Stakeholder Committee (Exhibit B) and a Project
Development Committee (PDC) (Exhibit C). The Stakeholder Committee will meet
monthly to allow collaborating agencies an opportunity to provide regular input on
the design, development, implementation potential, evaluation measurements and
program outcomes measures. The PDC will facilitate the work of the Stakeholder
Committee and the Transitional Board. The PDC will report to both the
Stakeholder Committee and the Transitional Board.
a. The Stakeholder Committee will approve L4H Guiding Principles as
developed by the PDC (Exhibit D)
b. The PDC will be responsible for the final draft of a White Paper (Exhibit E)
outlining the project. The Stakeholder Committee will approve final version
and send to the Transitional Board for adoption.
c. The PDC will ensure L4H maintains a timeline and work production based
on the Business Plan (Exhibit F).
d. The Stakeholder Committee will determine the feasibility of a demonstration
project, with the approval of the Transitional Board. L4H will continue
discussions with the State of Oregon regarding a formal Memorandum of
Agreement for a regional demonstration project to be signed by the
Transitional Board.
5. Provide web related updates to HealthMatters of Central Oregon in order to
maintain a neutral L4H working website.
6. State in all L4H marketing, advertising and print material for the L4H program that
the L4H members are collaborating stakeholders for this project. The L4H will
further acknowledge other lead partners as requested by the Stakeholder
Committee and the Transitional Board.
HealthMatters of Central Oregon (HMCO) agrees to:

1. Continue to be the neutral convening community hub and will work with other
mernbers of L4H to secure collaborative partnerships, grants and resources that
would stabilize and sustain the regional L4H demonstration project;
2. Provide technical assistance and support for the development of a regional L4H
website with collaborative registration capabilities. HMCO will ensure the L4H
website is up to date, and functional for the tri -County area and for the benefit of
the L4H collaborating agencies working on behavioral health integration.
3. Provide additional staffing support, as needed, for the L4H.
4. Provide a care coordination pathways program for the Stakeholder Committee to
review and integrate into the L4H demonstration project.
5. Provide a contract evaluator to help in establishing program evaluation
measurements and build a scorecard to make regional efforts, participant reach
and outcomes transparent to our community and funders.
IT IS MUTUALLY AGREED THAT:
Partner agencies will share program information for grant -specific reporting and writing,
as well as, ongoing budgetary and ancillary needs so grants can be tailored to regional
needs.
Partner agencies will collaborate and communicate with each other regarding the
marketing methods and promotion of L4H programs and resources.
Partner agencies will share data and information regarding the processes and outcomes
of conducting L4H demonstration project.
Partner agencies will explore and share methods of community outreach for expanding
L4H efforts.
Partner agencies will establish an effective process of communication and promptly
provide one another with information on program activities and outcomes.
Any partner agency may terminate its participation in the L4H project at any time upon
providing thirty (30) days' advance, written notice of termination to HMCO. Upon the
effective date of termination, the terminating partner shall have no further rights or
responsibilities under this MOU. However, the termination shall have no affect on the
validity of this MOU or the rights or responsibilities conferred by this MOU upon the
remaining participants in the L4H project.
Nothing in this MOU is intended or shall be interpreted to obligate any participating
agency to provide financial support to HMCO or the L4H project.

The L4H project and this MOU shall be performed and carried out in a manner that
complies with all applicable laws and regulations, including, without limitation, the Health
Insurance Portability and Accountability Act of 1996 and other laws and regulations
governing the use and disclosure of confidential patient information.
