OREGON COMMISSION ON CHILDREN AND FAMILIES 2009-2011 COUNTY
INTERGOVERNMENTAL AGREEMENT DES0911
This Amendment Number 6 (this “Amendment”) to Oregon Commission on
Children and Families 2009-2011 County Intergovernmental Agreement between the
parties hereto and dated as of July 1, 2009 (as amended from time to time, the
“Agreement”), is entered into as of the date of the last signature hereto, by and between
the State of Oregon acting by and through its Oregon Commission on Children and
Families (“Agency”) and Deschutes County acting by and through its Commission on
Children and Families (“County”).
RECITALS
A. WHEREAS, the Agency and County wish to modify the Award set forth in
Exhibit C of the Agreement.
B. WHEREAS, the Agency and County further wish to modify the Agreement with
respect to Healthy Start services to be provided hereunder.
AGREEMENT
NOW, THEREFORE, in consideration of the premises, covenants and agreements
contained herein and for other good and valuable consideration, the receipt and
sufficiency of which is hereby acknowledged, the parties hereto agree as follows:
1. The Award section of Exhibit C of the Agreement is hereby amended to read in
its entirety as set forth in Exhibit 1 attached hereto and incorporated herein by this
reference.
2. The Agreement is further amended with respect to Healthy Start services to be
provided by County as reflected in Exhibit 2 attached hereto and incorporated
herein by this reference.
3. County represents and warrants to Agency that the representations and warranties
of County set forth in Section 2 of Exhibit E of the Agreement are true and correct
on the date hereof with the same effect as if made on the date hereof.
4. Agency shall adjust the disbursement of the Award to County consistent with the
change in the Award made by Exhibit 1 of this Amendment.
5. Capitalized words and phrases used but not defined herein shall have the
meanings ascribed thereto in the Agreement.

EXHIBIT 2
HEALTHY START SERVICES
With respect to Healthy Start funding advanced to the County under the Agreement for
services to be provided by County for fiscal year ending June 30, 2011, the County will
provide a minimum of 60 family service units annually with Intensive Home Visiting
with respect to the provision of such services:
• Act reasonably to meet each of the 12 Critical Elements of Healthy Families
America (“Elements”) as referenced in the Agency’s Healthy Start Program
Policy & Procedure Manual;
• Act reasonably to meet all outcomes for the Healthy Start Performance
Indicators (“Outcomes”) in accordance with data reported by NPC Research
and Agency;
• If unable to meet each of the foregoing Elements and Outcomes, demonstrate
reasonable efforts to improve areas of underperformance
• Materially comply with all other applicable provisions of the Agency’s Healthy
Start Program Policy & Procedure Manual.
