Consideration and signature of Document # DC -2008-003 — the State Intergovernmental Agreemer.t #
121008 Revision 5 between the State Dept. of Human Services (DHS) and the Deschutes County Health
Department for access to Health care services.

BACKGROUND AND POLICY IMPLICATIONS:
The Oregon Department of Human Services administrates Federal Public Health Grant Funds, poolr,
these funds with some State Funds and then allocates these to local Health Departments. This is the
primary means State and Federal Public Health Funds are delivered to the local Health authority.
Document # DC -2008-003 - Agreement # 121008 revision 5 references the following revenue from
the State Dept. of Human Services:
(PE 12) State Bioterrorism Program Funds reduced:
1) ($24,005): Preparedness funds
2) ($12,794): Pandemic Flu funds
Total: ($36,799)
The entire State of Oregon received funding cuts to the Bioterrorism Programs due to Federal
funds being reduced. We expected the lost revenue due to funding forecasts earlier this year. Tie
Bioterrorism grant has been reduced by these amounts.
3) $60,000: (PE 42) for School Based Health Centers (SBHC) to fund Phase I Planning Grants or
$30,000 each to provide health care access to Bend -La Pine and Redmond School Districts. Funds
for this grant must be spent between December 1, 2007 and June 30, 2008.
Expectations for the planning grant involve engaging the community in school-based health
centers, meeting with community partners, and creating and implementing a collaborative strate ie
plan to develop, implement and maintain a school-based health center to serve school-age
children.
No staff will be added during Phase I, but some current School Based Health Center staff will bc
assigned to this project for part of their FTE or some additional hours. Some medical equipment
may be purchased to expend unused funds at the end of the grant.
Starting Date: July 1, 2007 Ending Date: June 30, 2008
Total Value: $23,201
Fiscal Implications:
For the School Based Health Center (PE 42): As this is a reimbursement grant, at the time the
County provides documentation to the grantor for expended resources up to the grant amount of an
additional $60,000 for Health School Based, the County will receive the grant resources.
For the Bioterrorism Grant (PE 12): Internal budget adjustments will be made for the Bioterrorism
grant award decrease — [Preparedness ($24,005), Bioterrorism - Pan Flu, ($12,794), totaling
($36,799)] - to not overspend the grant amount.
RECOMMENDATION & ACTION REQUESTED:
Approval and signature of Document # DC -2008-003, Intergovernmental Agreement # 121008
Rev. 5 between the Local Public Health Dept. and the Oregon State Department of Human Services is
requested.

FIFTH AMENDMENT TO DEPARTMENT OF HUMAN SERVICES
2007-2008 INTERGOVERNMENTAL AGREEMENT FOR THE
FINANCING OF PUBLIC HEALTH SERVICES
This Fifth Amendment to Department of Human Services 2007-2008 Intergovernmenta
Agreement for the Financing of Public Health Services, effective July 1, 2007 (as amended ill(
"Agreement"), is between the State of Oregon acting by and through its Department of Human Services.
("Department") and Deschutes County, acting by and through its Deschutes County Human Service
("LPHA"), the entity designated, pursuant to ORS 431.375(2), as the Local Public Health Authority for
Deschutes County.
RECITALS
WHEREAS the Department and LPHA wish to modify the set of Program Elements located in
Exhibit B "Program Element Descriptions" of the Agreement
WHEREAS, the Department and LPHA wish to modify the Financial Assistance Award set
forth in Exhibit C of the Agreement.
NOW, THEREFORE, in consideration of the premises, covenants and agreements contained
herein and other good and valuable consideration, the receipt and sufficiency of which is hereby
acknowledged, the parties hereto agree as follows
AGREEMENT
1. Exhibit B "Program Element Descriptions" is hereby modified as follows: Program Element
(PE) #12 named "Public Health Emergency Preparedness (PHEP) and Communicable Disease
Response Program" is hereby superseded and replaced in its entirety with the revised version of
PE 12 "Public Health Emergency Preparedness (PHEP) and Communicable Disease Response
Program" which is set forth in Exhibit 1 "Program Element Descriptions" to this Amendment.
2. Section 1 of Exhibit C entitled "Financial Assistance Award" of the Agreement is hereby
superseded and replaced in its entirety by Exhibit 2 attached hereto and incorporated therein the
Agreement by this reference. Exhibit 2 must be read in conjunction with Section 4 of Exhibit
C, entitled "Explanation of Financial Assistance Award" of the Agreement.
3. LPHA represents and warrants to Department that the representations and warranties of LPl IA
set forth in Section 2 of Exhibit E of the Agreement are true and correct on the date hereof with
the same effect as if made on the date hereof.

4. Capitalized words and phrases used but not defined herein shall have the meanings ascribed
thereto in the Agreement.
5. Except as amended hereby, all terms and conditions of the Agreement remain in full force and
effect.
6. This Amendment may be executed in any number of counterparts, all of which when taken
together shall constitute one agreement binding on all parties, notwithstanding that all parties
are not signatories to the same counterpart. Each copy of this Amendment so executed shall
constitute an original.
7. This Amendment becomes effective on the date of the last signature below.
