PUBLIC HEARING ON THIS DATE? No. BACKGROUND AND POLICY IMPLICATIONS: The Oregon Department of Human Services administers federal public health grant funds, pools these funds with some state funds and then allocates these to local health departments. This is the primary means state and federal public health funds are delivered to the local health authority. Document # 2008-505 - Agreement # 121008 revision 11 references the following revenue from the Oregon Department of Human Services: 1) $4,120.00 — Women, Infants and Children (WIC) Program — $3,474.00 of this money represents payment for travel reimbursement for the WIC staff to attend the Statewide WIC Conference in Portland in May 2008. The remaining $646.00 is for travel reimbursement for Michael Rossi and Brenda Wood to attend the TWIST software clerical training in Portland in June, 2008. Starting Date: July 1, 2007 Ending Date: June 30, 2008 FISCAL IMPLICATIONS: This money is already in the budget for this fiscal year. Agreement #121008 ELEVENTH AMENDMENT TO DEPARTMENT OF HUMAN SERVICES 2007-2008 INTERGOVERNMENTAL AGREEMENT FOR THE FINANCING OF PUBLIC HEALTH SERVICES This Eleventh Amendment to Department of Human Services 2007-2008 Intergovernmenta[ Agreement for the Financing of Public Health Services, effective July 1, 2007 (as amended the "Agreement"), is between the State of Oregon acting by and through its Department of Human Services ("Department") and Deschutes County, acting by and through its Deschutes County Human Services ("LPHA"), the entity designated, pursuant to ORS 431.375(2), as the Local Public Health Authority for Deschutes County. RECITALS WHEREAS, the Department and LPHA wish to modify the Financial Assistance Award set forth in Exhibit C of the Agreement. NOW, THEREFORE, in consideration of the premises, covenants and agreements contained herein and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties hereto agree as follows AGREEMENT 1. In accordance with the term "Agreement Settlement" contained in Paragraph 2. of Exhibit A, Section 1. of Exhibit C entitled "Financial Assistance Award" of the Agreement is hereby superseded and replaced in its entirety by Exhibit 1 attached hereto and incorporated therein the Agreement by this reference. Exhibit 1 must be read in conjunction with Section 4 of Exhibit C, entitled "Explanation of Financial Assistance Award" of the Agreement. 2. LPHA represents and warrants to Department that the representations and warranties of LPHA set forth in Section 2 of Exhibit E of the Agreement are true and correct on the date hereof with the same effect as if made on the date hereof. 3. Capitalized words and phrases used but not defined herein shall have the meanings ascribed thereto in the Agreement. 4.
