Describe the applicant organization, including its purpose, leadership

Describe the proposed project or activity.
We are seeking financial assistance in meeting the increase in need for our free
meal program. Our meal service grew 7.5% in the first half of 2014 as compared
to the first half of 2013.
Provide a timeline for completing the proposed project or activity.
The requested funds would cover the ingredient cost of approximately 2,000
meals. We are currently serving an average of 5,000 meals per month. We
anticipate these funds being used in the month of September, as our program
will be fully experiencing the effect of slower donations in the summer months.
Explain how the proposed project or activity will positively impact the
community.
When a person is hungry and unaware of where the next meal will be found for
themself and their family, it becomes an all-encompassing and immediate
search. We alleviate this concern so that people can spend their time and
resources on the next big issue.
Identify the specific communities or groups that will benefit.
Our diners are unemployed or under -employed, homeless or in low income
housing. They are all ages, from birth through old age. We serve those who are
experiencing food insecurity.
Describe how grant funds will be used and include the source and amounts
of matching funds or in-kind contributions, if any. Itemize anticipated
expenditures.
The funds will be used to purchase ingredients for approximately 2,000 meals.
Typical ingredients include: eggs, ground beef, cheese, peanut butter, lettuce
and assorted salad vegetables, broth, flour, sugar, milk, butter, etc. We receive
regular monthly donations from individuals equaling approximately $3,000.
If the grant will support an ongoing activity, explain how it will be funded
in the future.
We will continue to seek individual, church, business and grant funding. We
have been in operation for 28 years and maintain our goal of not turning
anyone away from our meal service.
Respectfully submitted by:
Donna Burklo, Family Kitchen Development & Marketing Director.

11 iic n Family Kitchen Fact Sheet
Guided by the belief that nobody should be hungry, the Family Kitchen has
been providing meals to anyone who is hungry for the past 27 years. Our
mission is "To serve anyone who needs a nutritious meal in a safe and
caring environment". Our patrons are treated with respect and are asked
nothing but to leave with a full stomach.
History
We began in 1986 when 6 people from Trinity Episcopal Church saw a need due
to the closing of the local lumber mills and hard economic times in Bend. They
prepared meals in their homes and served them once a week in Brooks Hall.
Other churches joined in and the meal schedule was expanded. Today the
Family Kitchen continues as an outstanding ecumenical effort serving about
5,000 meals a month. It operates as the Family Kitchen Trust led by its Board
of Trustees. (Tax ID #93-0427371)
Meals
In the past 3 years, meals served have increased from 2,500 to 5,000 a month.
Hot lunches are provided on Monday, Wednesday and Friday at 11:O0AM and
Saturday at 11:30AM. Hot dinners are served restaurant -style on Tuesday,
Thursday and Friday at 5:OOPM. Additionally, sack lunches are made on
Tuesday morning and are picked up by Central Oregon Veterans Outreach for
distribution at area homeless camps. In 2013, 55,970 meals were served - a 5%
increase over 2012 and a 19% increase from 2011. April 2014 saw our highest
meal count ever: 5,540 meals served.
Operations
Teams prepare and serve meals. Each team's leader is a certified food handler.
Other teams shop for food and pick up donated breads and desserts from local
markets. Our Program Coordinator obtains donated or reduced price meat and
canned goods from Neighbor Impact and purchases food from local grocery
stores. Food costs are about $1.00 per meal. Our 250+ monthly volunteers
come from churches, civic groups, schools, businesses and corporations.
Facilities
In 2010 a new kitchen was built on the ground floor of St. Helens Hall of Trinity
Episcopal Church. The kitchen is certified "commercial" and is subject to
regular county health inspections. The range was donated by Jan and Jody
Ward. The dining room is clean and cheerful, with seasonal table decorations
being provided by the Soroptomist Club. Patrons are served at tables with
placemats, silverware and napkins. A piano is available and is used frequently.
A lovely mural on one wall was painted by local artist Kim Smallenburg.

To Whom It May Concern:
RE: Non -Profit Status of The Family Kitchen
The Family Kitchen is a secular outreach program of Trinity Episcopal Church. The
entity "The Family Kitchen" was defined by resolution of the Vestry of Trinity
Episcopal Church as a trust of the church. It has its own by-laws, budget, and
accounting, and is governed by a Board of Trustees. The Board of Trustees is
responsible for the financial, strategic and tactical operation of The Family Kitchen. It
is accountable to the Vestry of the church as set forth in the by-laws.
All funds received by Trinity Episcopal Church designated for The Family Kitchen are
`restricted funds' and are administered by Board of Trustees of The Family Kitchen.
Because of the nature of the relationship of The Family Kitchen and Trinity Episcopal
Church, The Family Kitchen operates under the 501c3 of the Episcopal Church and the
EIN #93-0427371, which is held by Trinity Episcopal Church in Bend, Oregon.
