Organization Description: This organization is in the process of converting to the La
Pine Gap Network, one of five regional networks organized by the Partnership to End
Poverty. The program is a collaboration of churches and non -profits designed to
coordinate the delivery of social services by these organizations.
Project Name: La Pine Gap Network Program
Project Description: These funds will be used to coordinate the delivery of social
services to the region's underserved population, provide a local clearinghouse for people
in need of services, provide case management when necessary, uniting the churches and
non -profits, and encouraging volunteerism.

The LaPine Gap Network program.

1 Describe the applicant organization, including its purpose, leadership
structure, and activities.
A. This application is presented by the LaPine Community Action Team, which is in the
process of being converted to a DBA known as the LaPine Gap Network program. GAP
Network is a program of the Cascade Faith Based Network, one of five such regional
Networks organized by the Partnership to End Poverty.
B. The Gap Network leadership structure includes 1) Chris Riggs, LaPine Community
Kitchen, Board Chair of the Gap Network program, 2) Lori Weller, Executive Director of
the Gap Network program and 3) Jim Schell, the Development Director of the
Partnership to End Poverty and also the volunteer facilitator of the Cascade Faith Based
Network.
C. The LaPine Gap Network program is a collaboration of churches and nonprofits designed
to coordinate the delivery of social services by churches and nonprofits. Or phrased
another way, the program is designed to fill the gaps in social services that are currently
falling through the cracks. The Cascade Faith Based Network is made up of pastors, lay
people and nonprofit service providers working together to make the South County
region a better place to live.
2 Describe the proposed project or activity:
The primary activities of the Gap Network program include: 1) uniting the region's 22
churches in coordinating the delivery of a wide variety of social services to the region's
underserved population, 2) providing a local, volunteer -manned, Clearinghouse (phone-in
and walk-in center) for people in need to be directed to the services they require, 3)
providing, where necessary, case management for those people in need, 4) uniting the
nonprofit sector and the faith -based sector in coordinating the delivery of social services
and 5) encouraging social -service volunteerism by the member -church's congregations.
The Clearinghouse will be located in the St. Vincent DePaul building in LaPine. It will be
manned by community volunteers, many of whom will presumably come from the region's
church membership. St. Vincent DePaul will provide the office space and is an active partner
in the program.
The model for this program can be found at www.loveinc.com in Newburg, Oregon. The
Newburg model involves 44 churches working together. A similar Gap Network program is
also scheduled to begin in Madras in January.
3 Describe a timeline for completing the proposed project or activity.
The Gap Network program is scheduled to officially roll out in late February of 2012. As of
this writing, the exact date for the kick-off Open House has not been established. The
program is an ongoing project and, once up and running, will not have an ending date.
4 Explain how the proposed project or activity will impact the
community's economic health.
The direct economic impact will depend upon the number of clients that the Call Center
services and the focus of those services that are requested. Where those inquiries involve
job related services (such as training or transportation), or education resulting in attaining
jobs, or healthcare resulting in abetting the retention of current jobs, the economic impact
is evident. Where those inquiries involve a long list of social services that provide emergency
lifestyle resources (food, clothing, shelter) the economic impact is less tangible, but
nevertheless real.
Finally, the GAP Ministry program will introduce new volunteers into the social delivery
scene, in effect bringing more services to the under -served. This increase in volunteer hours
represents either cost savings to the public sector or increased value to South County
citizens or both.
5 Identify the specific communities or groups that will benefit.
The Gap Network program will serve all of the low-income and/or underserved population
of the South County.
6 Itemize anticipated expenditures. Describe how grant funds will be
used and include the source and amounts of matching funds or in-kind
contributions, if any. If the grant will support an ongoing activity,
explain how it will be funded in the future.
A. Budget:
Startup Expenses: Office Equipment, phone and internet, $1,000
leasehold improvements, marketing materials, etc.
Annual Operating Expenses: Utilities (phone & internet), $1,800
marketing, gas & travel, etc. @ $150 per month.
Services Provided: This line item could result in just about any TBD
amount. It will depend upon how much money is raised. Services
provided would include such expenditures as gas vouchers, energy
vouchers, transportation vouchers, overnight living vouchers, and a
wide variety of related emergency services and products. The exact
amount will depend upon the availability of funds.

C. Sustainability:
If other similar programs (Newberg, McMinnville, Ontario, for instance) are any
indicator, future funding will come from foundation grants, church fundraising,
and other regional and local supporters.
