Prepared by Jim Schell 1 Describe the applicant organization, including its purpose, leadership structure, and activities. A. This application is presented by the Bend's "Bridging Gaps" program. The Bridging Gaps program is currently being developed and is scheduled to go live in late summer of 2012. The Bridging Gaps program is, in turn, a subsidiary program of the Bend Faith Based Network, one of five such regional Networks organized by the Partnership to End Poverty (PEP). Partnership to End Poverty is not officially connected with the Bend FBN, but does provide volunteer facilitation, organizational assistance and a small amount of funding. The fiscal agent for this grant is the Community of Christ Church, represented by FBN Network members Craig and Tami Perryman. B. The Bridging Gaps program includes in its leadership structure: Tami Perryman, Community of Christ church Lisa Radford, United Methodist of Bend church Carolyn Klindt-Stoops, First Presbyterian church C. The Bridging Gaps organization represents a collaboration of churches and nonprofits whose goal is to coordinate the delivery of social services by Bend's churches and nonprofits. Or phrased another way, the program is designed to fill the gaps in social services that are currently falling through the cracks. The overseeing organization - the Bend Faith Based Network - is made up of Bend pastors, lay people and nonprofit service providers working together to make our community a better place to live. 2 Describe the proposed project or activity: The primary activities of the Bridging Gaps program include: 1) Uniting Bend's churches in coordinating the delivery of a wide variety of social services to the community's underserved population, 2) Providing a local, volunteer -manned, Clearinghouse (phone-in and walk-in center) for people in need to be directed to the services they require, 3) Providing, where necessary, case management for people in need, 4) Uniting the nonprofit sector and the faith -based sector in effectively coordinating the delivery of social services and 5) Encouraging social -service volunteerism by the member church's congregations. The location for the Clearinghouse has not yet been determined but will be within close proximity of downtown Bend. The Clearinghouse will be manned solely by community volunteers, many of whom will presumably come from Bend's church membership. It is hoped that the facility for the Clearinghouse will be donated by one of the churches. The model for this program can be found at www.loveinc.com in Newburg, Oregon. The Newburg model involves 44 local churches working together. A similar "Gap Network" program was rolled out in Madras in January, 2012. Also, a similar program is also in the startup phase in LaPine. 3 Describe a timeline for completing the proposed project or activity. The Bridging Gaps program is scheduled to go live sometime in the late summer or early fall of 2012. As of this writing, the exact date for the kick-off Open House has not been established. The program represents the beginning of an ongoing project and, once up and running, will not have an ending date. It is primarily volunteer -driven and fixed costs are low , which means sustainability should not be an issue. 4 Explain how the proposed project or activity will impact the community's economic health. The direct economic impact will depend upon the number of clients that the Call Center services and the focus of those services that are requested. Where those inquiries involve job related services (such as training or transportation), or education resulting in attaining jobs, or healthcare resulting in abetting the retention of current jobs, the economic impact is evident. Where those inquiries involve a long list of social services that provide emergency lifestyle resources (food, clothing, shelter) the economic impact is less tangible, but nevertheless real to those who look to the social service community to solve their problems. Finally, the Bridging Gaps program will introduce new volunteers from the churches into the social delivery scene, in effect bringing more services to the under -served. This increase in volunteer hours represents either cost savings to the public sector or increased value to Bend area citizens or both. 5 Identify the specific communities or groups that will benefit. The Bridging Gap Network program will serve all of the low-income and/or underserved population in Bend, with a secondary goal being the same population from surrounding areas of Deschutes County. 6 Itemize anticipated expenditures. Describe how grant funds will be used and include the source and amounts of matching funds or in-kind contributions, if any. If the grant will support an ongoing activity, explain how it will be funded in the future. A. Budget: Startup Expenses: Office Equipment, phone and Internet, $1,000 leasehold improvements, marketing materials, etc. Annual Operating Expenses: Utilities (phone & Internet), $1,800 marketing, gas & travel, etc. @ $150 per month. Services Provided: This line item could result in just about any TBD amount. It will depend upon how much money is raised. Services provided would include such expenditures as gas vouchers, energy vouchers, transportation vouchers, overnight living vouchers, and a wide variety of related emergency services and products. The exact amount of vouchers issued will depend upon the availability of funds. B. Source of other funds: 1. Partnership to End Poverty grant 2. Commission on Children & Families grant $1,500 $1,500 C.
