Fair and Expo Director Dan Despotopulos and Mary Goodwin, emergency preparedness coordinator in the Health Services Department, will be at your May 25`" work session to brief you on these efforts. A Federal Medical Station (FMS) helps support overwhelmed communities in a mass casualty event by providing beds, supplies, medicine, medical and logistical support. It must have the capacity to treat up to 250 people for three days. An FMS scenario would look like this: St. Charles Hospitals in Bend, Redmond, Madras, and Prineville are full, and a surge of acutely ill or badly injured patients are expected. The hospitals will release their sub -acute patients to be transported to the FMS where medical services would be provided. The regular hospitals would then attend to only the most acute cases. A disaster triggering an FMS would be of such magnitude that a state and/or federal emergency would be declared. An MOU between the Fair and DCHS would expedite the process for both requesting and receiving an FMS and facilitating a much more rapid response in that critical time rather than permission -seeking and negotiating details in such an event. Enhancing the Lives of Citizens by Delivering Quality Services in a Cost -Effective Manner Deschutes County Health Services Memorandum of Understanding for A Federal Medical Station (FMS) Site in Deschutes County This Memorandum of Understanding ("MOU") is entered into by and between Deschutes County Fair and Expo ("DCFE") and Deschutes County Health Services ("DCHS"). The purpose of this MOU is to set forth the expectations of the parties in the event of a large-scale disaster where medical services beyond the capacity of local hospitals can be furnished. Throughout this MOU, the term, FMS is used. An FMS is an asset of the Federal Emergency Management Agency which includes medical and logistical staff, supplies, and equipment to treat up to 250 sub -acute patients. 1. DCFE hereby acknowledges its intent to provide facility space and services associated with designated portions of the Deschutes County fairgrounds acting as an FMS site in the event of a large-scale disaster whereby local hospitals have exceeded or are expected to exceed their capacity to treat anyone except the most severely ill or injured patients. Less acute patients would be moved or directed from local hospitals and health care providers to available sites in Deschutes County, designated as FMS sites. In making this acknowledgment, DCFE will endeavor to do the following should a large scale medical emergency occur, and DCFE is requested by Deschutes County Health Services ("DCHS") to provide assistance as outlined in this MOU: 1.1. To permit, if and to the extent practicable and upon request of DCHS, the use of designated DCFE physical facilities and designated equipment by DCHS as soon as possible but ideally within 24 hours of the request and for the requested time period, for the FMS. The parties understand that availability of facilities and equipment is subject to possible commitment or booking for or on behalf of another organization or event. 1.2. To designate the following three points of contact in case of emergency and DCFE is called upon to open the FMS at the fairgrounds: • An administrator, who would serve as the primary point of contact between DCHS and DCFE. This person shall have authority to open up designated DCFE facilities. • A facilities point of contact, who would coordinate directly with DCHS personnel, including volunteers to arrange physical space and to provide other equipment, supplies and services as needed and available. • A security point of contact, who would coordinate with the DCHS and local law enforcement or the particular incident command center in carrying out security plans. 1 Deschutes County Health Services 1.3. To allow designated DCFE facilities to be visited by DCHS, local law enforcement and if applicable, the National Guard, for the development of the FMS. 2. DCHS hereby acknowledges its responsibility to: 2.1. Provide a DCHS point of contact to provide coordination of resources, to respond to questions and to address issues and concerns that may arise in the event of the opening of the FMS at the fairgrounds. 2.2. Replace or reimburse DCFE for any DCFE supplies or materials that may be used by DCHS in the operation of the FMS under this MOU. Assist DCFE in making application for payment for use of DCFE facilities and compensation for any losses due to displacement or cancellation of other organizations or events by the FMS. 2.3. Provide health and/or security professionals to meet people at the entrance of the FMS facility used pursuant to this MOU. 2.4. Coordinate the provision of extra security personnel, if needed. 2.5 Provide any needed post -event cleanup of DCFE facilities. 2.6. Provide training for personnel who will staff the FMS. 3. The parties further agree to the following: 3.1. No permanent modifications or changes will be made to any DCFE facilities or property without the express approval of the Fair Board. 3.2. It is understood that DCFE will maintain, and will not relinquish operational control of the fairground, but will remain flexible in making arrangements that will minimize the disruption that serving as an FMS site could entail. 3.3. This Agreement shall be effective upon approval by the parties and shall remain in effect unless terminated as provided in this paragraph. Any party may terminate its participation in this Agreement as follows: Written notice of intent to terminate this Agreement must be given to the other party no fewer than 90 days prior to termination date. A disaster triggering an FMS would be of such magnitude that a state and/ or federal disaster would be declared. An MOU would expedite the process for both requesting and receiving an FMS, facilitating a much more rapid response in that critical time rather than permission -seeking and negotiating details in such an event. i �1STI}SI�N OSP TI © AL Federal Medical Station Profile State and local health resources can quickly become overwhelmed in the event of a disaster. CDC's Division of Strategic National Stockpile (DSNS) can assist these communities by deploying Federal Medical Stations, or FMS. An FMS is a cache of medical supplies and equipment that can be used to set up a temporary non-acute medical care facility. Each FMS has beds, supplies, and medicine to treat 250 people for up to three days. The local community will probably be required to provide some operational support. A 250 -bed FMS set consists of three modules: • Base Support : Administrative, food service, housekeeping, basic medical supplies, and personal protective equipment. There are five bed units, with 50 beds each. • Treatment : Medical/surgical items • Pharmacy : Medications Technical teams from DSNS deploy with the FMS sets to provide set-up assistance. The stations are operated by federal, state and local groups. Most of the items in the set are expendable, with only a few recoverable items. Currently available FMS sets can be used for inpatient, non-acute treatment. Department of Health and Human Services and the Department of Homeland Security are developing other models which will provide critical care and special needs capabilities. State and local officials will locate and determine the suitability of an existing facility or structure that can be used as an FMS site. One FMS set requires a minimum of 40,000 square feet of enclosed, climate controlled space with the following required features : • Adequate loading ramps, materiel handling equipment (MHE), and parking • Sufficient existing communications/IT support and power supply (with back-up) • Support services including, food and water, waste disposal, medical oxygen, laundry, mortuary services, etc. • Refrigeration and controlled substance storage • Bathrooms and shower capability • Billeting support for 150+ FMS staff • Security * These are the minimum requirements for an FMS site to be functional; contact your State DSPVS representative for detailed requirements for your planning. Telephone: (404) 639 - 0324 1 EMERGENCIES ONLY: (770) 488-7100 (cont) FMS At A dance FMS helps support overwhelmed communities in a mass casualty event by : • Quickly turning a building of opportunity into a temporary medical shelter • Accompanied by DSNS/FMS logistical team for set up support • Stocked with beds, supplies, and medicine to treat 250 patients for up to three days • Can be used for inpatient, non-acute treatment • Modular Configuration • Scalable according to size of incident • Modeled for all age populations • Transports by air or ground for rapid, maximum geographic distribution FMS and Hurricane Relief When Hurricane Katrina struck Louisiana on August 28, 2005, only a few prototype Federal Medical Stations existed. DSNS took the program from prototype to reality almost overnight. Over the next few weeks, DSNS sent nine FMS sets with 5,500 beds to hurricane -affected areas. DSNS deployed technical teams to facilitate set up of these sets. FMS equipment and supplies were sent to Louisiana, Mississippi, Texas, and Florida to assist local public health teams with hurricane relief.
