Project Name: Community Exploration — Deschutes County Fair Project Description: This grant will be used to pay for admission to the 2011 Deschutes County Fair for 45 clients receiving services from the Community Support Team. Previously, clients were taken to the fair on free days, which are not available this year. Project Period: August 2011 Amount of Request: $450 Previous Grants: • June 2010: $360 Fair Tickets ($360 Luke) To: DCBOC From: Community Support Services, Mental health Date: June 20, 2011 HEALTH SERVICES 1128 NW Harriman Street • Bend, Oregon 97701 Phone (541) 330-4637 • FAX (541) 330-4642 www.deschutes.org I would like to request $450, to be used to get 45 low income individuals who receive mental health services from the County, into the Deschutes County Fair for the 2011 season. In the past we have made a real effort to use the County Fair free day or have been awarded Grant money from the County, to be used for this purpose. A day at the County Fair is something our Clients look forward to every year. It helps them to partake in a healthy community activity which promotes community integration for individuals who are not only economically underprivileged but also isolated and struggling with a multitude of serious mental health challenges. This activity would be accompanied by mental health professionals employed by the County. This is because going to a community event can trigger symptoms hence staff involvement is helpful. Their would be numerous groups going ( Clubhouse, Horizon House, Emmas Place , Men's grp, Foster Homes) with a ratio of one staff to 8 clients. Those using the tickets would be adults. Please consider this a valued way to support some of the citizens of this County. We have greatly appreciated your generosity in the past and hope you will support this request. Thank you. Suzanne Brock, QMHA, will be the contact person for this request.
