BACKGROUND AND POLICY IMPLICATIONS:
The purpose of this amendment is to update funding for the Early Learning Hub and Maternal Child
Health Program. This amendment provides the Early Learning Hub $105,326 in equal monthly
installments until June 30, 2015 and the Maternal Child Health Program $88,000, divided into two
(2) payments; first $66,600 and the second for $22,200.
The Central Oregon Health Board is a 190 organization formed by Crook County, Deschutes
County and Jefferson County to improve health outcomes, increase satisfaction with the health
systems and reduce healthcare costs. The goal of the Wellness and Education Board of Central
Oregon (fka Central Oregon Health Board) is to manage resources efficiently and effectively, in
collaboration with local and State Government, the hospital system, local providers, private
insurers, health collaboratives and most importantly the community and the people Deschutes
County serves.
A number of health organizations and interests in Central Oregon have been working to develop a
structure and set of functions to improve the health of people in the Central Oregon region and the
performance of the health care system in Crook, Deschutes and Jefferson Counties. Represented
organizations include all three (3) counties (Boards of Commissioners), Deschutes County Health
Services (DCHS), PacificSource, St. Charles Health System, HealthMatters, Volunteers in
Medicine, Oregon Health Authority, BestCare Treatment Services, Lutheran Community Services
NW and others. In July of 2010, the Boards of County Commissioners (Boards) in all three (3)
counties agreed in concept that the region would benefit from a formal process for the
aforementioned groups to work together more closely in adapting to health reform changes at the
national, state and local levels.
WEBCO contracts and administers contracts with the State of Oregon and any other public and
private entity to provide or secure a wide range of health services including behavioral health,
physical health and public health. In addition to the administration of contracts, the Board:
a. Conducts a periodic regional health assessment to inform decision-makers on such matters
as planning, policy development, system design and investments.
b. Develops and establishes policies that support Deschutes County Health Services on
behalf of the Central Oregon population.
c. Improves the coordination of funding for Deschutes County Health Services among the
counties and relevant stakeholders. This includes establishing financial systems,
maintenance of financial reserve requirements, policies, and monitoring sub-capitated funds
of the counties and other Health Service providers in Central Oregon.
d. Develops and adopts a coordinated regional Health Improvement Plan to improve
collaboration among health care providers.
e. Collects and analyzes health system costs and outcome data to measure the quality and
performance of the health system in order to lower costs, increase access, improve
satisfaction and increase efficiency. This includes establishing management information
systems to meet reporting requirements.
f. Promotes evidence-based practices and prevention programs, including, but not limited to:
quality assurance guidelines and oversight, utilization management for services both
inpatient and outpatient, and providing member services including education and
prevention activities.
g. Promotes models of care that improve the provision of services for patients among various
providers so that individuals can navigate service systems successfully. This includes the
development and implementation of processes for hearing, monitoring and resolving
complaints and grievances.
h. Hire employees to perform administrative tasks and other duties as assigned or delegated
by the Governing Board and Administrative Council.
Through the formation of WEBCO, the counties are able to provide the best possible behavioral
health, physical health and public health services to the client population of all three (3) counties.
FISCAL IMPLICATIONS:
Maximum compensation is $193,326.

AMENDMENT NO.3
TO WELLNESS AND EDUCATION BOARD
OREGON INTERGOVERNMENTAL AGREEMENT
This Amendment NO.3 (Amendment) to the Wellness and Education Board of Central
Oregon Intergovernmental Agreemerit (Agreement) is entered into as of the date of the last
signature hereto.
AGREEMENT
A. An additional paragraph No. 31 is added to the original Agreement. The
original agreement was for CCO funding, but new funding has come along
and the parties agree to amend the contract for the new funding. The
Agreement is to read as follows:
Early Learning Hub. The parties agree that the Early Learning Hub
deliverables which are attached hereto as Exhibit "A" and incorporated herein
by reference have been adopted by the respective parties and shall be adhered
to by the parties in the future operations of Well ness and Education Board of
Central Oregon (hereinafter WEBCO). The attached Early Learning Hub
deliverables may be amended from time to time by the parties and such
amendments shall be in writing, signed by both parties, and attached to this
Amendment. Thereafter, such Amendment shall be deemed incorporated
herein without necessity of further amendment to the original Agreement.
Maternal Child Health. The parties agree that the Maternal Child Health
deliverables which are attached hereto as Exhibit "B" and incorporated herein
by reference have been adopted by the respective parties and shall be adhered
to by the parties in the future operations of Wellness and Education Board of
Central Oregon (hereinafter WEBCO). The attached Maternal Child Health
deliverables may be amended from time to time by the parties and such
amendments shall be in writing, signed by both parties, and attached to this
Amendment. Thereafter, such Amendment shall be deemed incorporated
herein without necessity of further amendment to the original Agreement.
B. Counterparts. This Amendment NO.3 may be executed simultaneously or in
counterparts, each of which shall be deemed an original, but all of which together will
constitute one and the same contract.
C. Reaffirmation. Except as amended herein, all terms and conditions of the
Agreement dated October 22,2012, as amended by Amendment NO.1 which was executed on
February 26, 2014, and Amendment NO.2 which was executed on August 27, 2014 shall
remain in full force and effect.
D. Effective Date. The parties hereto agree that the effective date of this
Amendment NO.3 shall be retroactive to January 1, 2015 and shall expire on June 30, 2015.

IN WITNESS WHEREOF, the parties have read this Amendment NO.3 to the
Agreement dated October 22, 2012 in its entirety, agree to it and hereby assert that they have
the authority to bind the respective parties to it.

EXHIBIT A
to Amendment No.3
Early Learning (EL) Hub Deliverables for the current period October 1,2014 to June 30, 2015:
Deschutes County has two individuals delivering services within the EL Hub. Deliverables are
described per position:
• Early Learning System Development Manager
o Provides administrative and operational support for the Early Learning Hub.
o Leads the development and implementation of EL Hub Strategic Framework and
action plans to accomplish strategic goals with support from the System Dev.
Specialists.
o Oversees the establishment of targeted outcomes and benchmarks with consistent
evaluation metrics and with support of WEBCO Data Analyst and System Dev.
Specialists.
o Assists in the creation and implementation of long-term funding strategy to
expand financial and volunteer capacity to support local and regional EL Hub
goals.
o Ensures compliance with all state and federal mandates, including required
reports to the Oregon Early Learning Division and Council.
o Supports, coordinates and/or facilitates meetings and activities of the Early
Learning Leadership Council, Executive Council, and the Central Oregon Early
Childhood Collaborative in partnership with Early Learning System Dev.
Specialists.
o Assists the Executive Council and Early Learning Leadership Council in building
and managing a strong, cross-sector membership and network of providers I
stakeholders.
o Identifies and engages with strategic partner organizations at the local, regional
and state levels. Includes promoting an integrated and efficient provision of
services among the EL Hub network of providers.
o Serves as primary contact person for ODE, ELD, ELLC, and AC.
o Assists the WEBCO ED with professional development planning and the EL
System Development Specialists with prioritized regional training initiatives.
• Early Learning System and Contract Development Specialist
o Supports development and implementation of EL Hub Strategic Framework;
includes developing and executing local and regional action plans to accomplish
strategic goals.
o Coordinates program and system development at the local and regional leveL
o Connects local strategies and outcomes with EL Hub strategies and outcomes.
o Assists with and coordinates local and regional trainings and education.
o Develops and leverages resources, including grant writing and program budget
preparation.
o Facilitates the development of perfonnance-based contracts:
• Establishes program/project goals, outcomes, perfonnance measures and budgets.
• Monitors/measures perfonnance toward outcomes and program expenditures.
o Compiles data, submits program and expenditure reports.
Compensation will be $105,326 and will paid in equal monthly payments through June 30, 2015.

MATERNAL CHILD HEALTH PROJECT EXTENSION
Services to be provided under this amendment:
• Continue systems integration work with primary care providers to enhance integration,
increase communication, and enable new and more efficient work flow processes all in
support of increasing capacity for regional services for high risk women and families
within the tri-county area.
• Establish new partnerships with primary care (St Charles Center for Women's Health)
providing (8 hours per week) public health maternal health educator and establishing
referral processes.
• Share Public Health outreach best practices learned using the embedded model through
educational sessions with community partners.
• Continued partnership with other community home visiting programs (Healthy Families
and Early Head Start) to coordinate services, improve continuum of care and establish
regional referral process in coordination with the Early Learning Division and Central
Oregon Early Learning Hub.
• Continue partnership with COHC, CCO and public health to identify long-term
sustainable funding strategies and/or payment methodologies through transition of
Medicaid Targeted Case Management in support of expanded nurse home visiting
programs.
System-wide outcomes for this six month continuation period December 31, 2014 to June 30,
2015 will include:
• Sustainable regional Nurse Home Visiting Services (Nurse Family Partnership and
CaCoon) as the transition occurs to a new funding model beginning in July 2015.
• Strong and sustained partnership with primary care (St Charles Center of Women's
Health), community home visiting programs as evidenced by more efficient and
streamlined referral processes across the region.
• Forward progress on the contract agreement between COHC, CCO and public health
outlining long-term, sustainable funding strategies and payment methodologies through
transition of Medicaid Targeted Case Management.
Compensation:
• Total compensation will be $88,800. It will be divided into two payments; the first for
$66,600 and the second for $22,200.
