Introductions & Approval of September Meeting Minutes
The Board went around the room and introduced themselves to guests and the minutes were
postponed until the next meeting due to lack of a quorum.
Brief Announcements & Unfinished Business
Tom will be emailing the meeting dates for 2018 and announced that Kate and Keith have
extended their terms, which by consensus, was approved after Tom emailed the Board.
Stacey Witte and David Huntley have applied to PHAB to become board members and will be
interviewing today after the monthly meeting. As per standard procedure, a letter will go out
to the BOCC for their approval.
Tom and Keith worked on a letter addressed to the Deschutes Collaborative Forest Project in
support of their smoke management practices. The BOCC approved of the idea so the letter
was sent off to Pete Caligiuri from the Nature Conservancy. Pete and Keith have been keeping
in touch and Pete may come speak to the Board again sometime in the Spring.
Regional Perinatal Care Project Overview
Erin Hoar, Regional Perinatal care Coordinator, gave a project update through a PowerPoint
presentation. Since 2000, Oregon Mother’s Care has been operating as a state run project
providing expecting mothers with prenatal care that has now expanded to the tri-county
region. Local key partners started discussions about creating a regional perinatal care model
back in Spring 2015. Their proposal was then pitched to the COHC in the summer of 2016 and
a 3 year grant was awarded in the fall. The project serves to expand high risk nurse home
visiting services, a link to community resources and coordinating/tracking pregnant women in
the tri-county area, which aids in increasing early access to care, a reduction in low birth
weight births and tracking of services by care coordinators. The ideal goal is what is best and
easiest for the client. As part of the PCC Model, DCHS has hired two new perinatal care
coordinators. Erin travels to both neighboring Jefferson and Crook counties to coordinate
efforts with their teams, as perinatal staff are the first point of contact for all pregnant women.

Erin spoke about the referral method using an assessment screening to determine eligibility,
which in turn starts the tracking process and referrals to other services. If a client is on OHP,
then they are automatically eligible for WIC, both of which are a huge part of the PCC. Erin
noted we are collecting data through various avenues: creating cards/surveys for client use,
word of mouth and building relationships with providers and clinics. Kate chimed in and said
she could include the link to our PCC site on PacificSource’s website. Jenny Faith worked on
regional demographics which were provided in Erin’s presentation. Erin also noted some
success stories which include an increase in WIC caseloads, a new developed project tool, and
increased integration with Advantage Dental, just to name a few. Other related activities
include a developed and shared training manual, creating internal/external workflows, outcome
data collection, the creation of new outreach materials, satisfaction surveys and outreach to
community partners. Our next steps will be to request a sustainment plan at year 3, develop
PCC program branding and marketing materials, and utilize PCC as a nurse home visiting
extender. Erin posed the question how PHAB may be able to support this program.
Retreat Results Review & Moving Forward
General Comments:
Tom is setting up the agenda of topics for 2018.
Review of what has been done and our focus for 2018.
Immunizations are way down on infants (600 cases of pertussis in 2017). Tom will send a link
to Dave Huntley of the immunization chart rates for schools as requested.
Create work groups or written format on how PHAB symbolically addresses a topic.
List of identified priorities.
Climate change and air quality will be considered as one topic of discussion.

Tom will work on identifying and assigning staff who specialize in the above mentioned topics
and have them work with PHAB leads during workgroup meetings.
Keith and Tom will draft a tobacco retail license letter to be reviewed by the BOCC. Penny
Pritchard (Tobacco Cessation Health Educator) is leaving her position to pursue other
prospects, so Tom will take lead on the project.
Action Items:
Send PHAB links to County school immunization rates
E-mail out info about Dr. Shirtcliff
Work to assign staff who could assist with priority areas
